No, I do not accept any insurance or Medicaid at this time. If you are covered by Medicaid, you must see a Medicaid provider.

Some insurance plans offer out-of-network reimbursement. If this is the case with your insurance plan, I can provide you a monthly "superbill" to submit. Call your insurance provider to find out if this is an option for you. I do not work with insurance companies directly. 

​Mentaya and Thrizer both have options for OON submission, but not required. While I do not work directly with these companies, I provide them as options for folks who find submitting claims to their insurance companies difficult. I receive no incentive for sharing these options, and there may be a fee incurred for using these programs. 

You may also use your HSA/FSA card to pay for services.
